Question to Lightroom users

I have started using Lightroom accepting the default paths.
Now I have my C drive full of those DNG files and want to migrate them and the "catalog" (which I assume is the settings) over to a portable HD.
The question is: how do I do that in a way that Lightroom will "know" where to look for the files and use the new directory from now on?
