Where can I get some 6x9 ground glass? My KW Reflex Box came with a broken (in half) piece.) I'd like to replace it for safety.

Nevermind. Calumet has a Cambo glass that should fit.

If that does not work out for you, I've had good experiences with hopfglass.com. They refinished a few custom pieces for me (out of old Ikoflexes) and did a great job.
