It has a movement to tilt at the focus mechanism. You have firmly the rear of the lens with one hand, and with the other you move the front of the lens, it wil move (not the rear) in the tilt form.
The focus mechanism seems to be worn away or so.
How can I repair this?

No a tilt lens, nor moved as it.
I said it very bad. 
When you focused the lens, it moved 1 or 2 mm. because the screw were bad, it's an old one. It didn't a tilt lens.
