no personal experience, good references from forums tho.

I've had two jobs done: Canon P shutter repair - did an excellent job - and a CLA and element re-centering on a 100/f3.5. Reasonably priced, fast turnaround.

Gets my vote.

Professional Camera Repair

I wanted to mention these good people: http://professionalcamerarepair.com/

Professional Camera Repair, 4410 Richmond Ave, Houston, TX 77027, USA, 713-621-3627

I sent in a number of old cine lenses for CLA. The man did great work. I sent in two Hugo Meyer cine lenses that were clouded. He cleaned them up, lubed the mechanism, and they are pristine. A few other lenses with jammed focus he unjammed.

I'll second Lyle's experience with Professional Camera Repair. I had them do a CLA on my Pentax LX. Cost was $80, hundreds less than the well-known Pentax repair guy that everybody over at the Pentax forums recommend.

I live in Houston, which is where they are located. This is the place the local pros use when their gear needs servicing. They're fast, they do good work, and they're reasonably priced.
