Well assuming good film and lens and a non faulty camera....it's usually:-

Poor scan or stale developer........In theory a 35mm colour neg film shot should equal a top 35mm DSLr as it contains about 20mp of detail, but the problem these days is how to get all this detail of cheaply.

If developer process has faulty, old over used developer for example (pretty common these days) film can be grainy, colors are ruined and I think sharpness also less.
