I don't understand how this focuses?

I understand that the lower box is purposed for storing negative glass plates

The viewing and taking lenses are recessed behind the front board, and focus is probably done internally, sliding the lens board back and forth within the camera.

On the contrary, according to the diagram it looks as though the knob at the rear of the camera moves the film plate backwards and forwards. The lens appears to be fixed.

The diagram is incomplete -- the lens panel sliding mechanism is missing. In the second photo, above the plate handler knob, you can see a heavy wire loop with the wire going through the back wall and disappearing between two horizontal panels towards the lens panel. The internal structure of the camera must be more complex than depicted in the diagram. The lens panel is probably the bottom of a sliding box with the walls sliding between double panels in the camera body. The walls of the box provide support for the lens panel and prevent light leakage.
