I bought a M42 lens from eBay. It came with this adapter.
A M42 lens can be mounted on the adapter.
What camera is this adapter for?
Is there a camera called "Pentax-S" that accepts this adapter through the 3 holes around its flange?
How is the adapter attached to the camera body?

Looks like a flange for a bellows or something similar.
There are some bellows that accepted different lens mounts.

There were also some cameras (Exaktas for one) where one could change the lens mount to M42 with a device like this - though this particular one isn't for Exaktas, those would have four screw holes.
