There is a seller "prolenscla" on ebay from Taiwan who offers those services incl. re-cementing: http://www.ebay.com/usr/prolenscla?_trksid=p2047675.l2559
Quote him: "Charge for lens group separation re-cementing treatment and damaged coating polishing treatment starting from US$ 150.
The actual charge depend on which kind of the lens and the size of the lens need to repair. It is necessary to negotiate between you
and me before sending me your lens." So I estimate it to be quite more actually... Wink

On his FB site https://www.facebook.com/CLAmanualfocuslens he shows some impressive results, I must admit.

Hi, this guy is actually amazing. And despite what the original post here might suggest he is charging a very reasonable amount. I paid 150 for getting a separated element in my Carl Zeiss Vario Sonnar 28-85 fixed (Plus he probably cleaned it generally too, came back like new). The lens works perfectly now, and i do have to say there are very few people who will tackle such a zoom! It has 16 elements, so surely no easy task.

Waiting time was rather substantial, about 2 months from him receiving to it being fixed. With shipping it took 3 months. But he does state on his website that he gets a lot of lenses to work on so completely understandable.

BTW: his name is Lin Wea Lun, the website can be found under http://lens-cla.blogspot.com/p/blog-page_19.html
