Its mount and aperture housing. Schneider produces certain lenses in multiple variants, Robert O Toole shows a Componon-S 50/2.8 here in makro iris and M39 versions:

The Makro Iris V38 mount has an aperture indicating 1 to 6 rather than standard f/stops. It also has a very sturdy 38mm diameter groove mount, both for the front and the rear mount. You do need specific adapters to mount it to more standard M39 or other mounts, those can be a bit hard to find and not all adapters mount onto all V38 lenses, some V38-M39 adapters (even original Schneider ones) are too narrow to mount onto V38 lenses with large rear lens groups like the Makro Symmar 120/5.9. The Makro Iris lenses are all metal construction, some M39 versions have more plastic barrels.
