This is a Telyt 200mm. f/4 originaly made for the Visoflex system. This is the high quality and well regarded f/4 Telyt and not the f/4.5 one, considered less good. This lens is adapted to the Pentax 645 mount and fully cover the 6x4.5 format. They say that Visoflex lenses were initialy designed for the large format cameras and then used for the Visoflex system. Anyway, all the Visoflex lenses cover well 645 format, without vigneting or smeared corners. This is a very sharp lens witha nice Bokeh. It is much better than the Pentax 645 own 200mm lens. I had them both and could compare them.

See here on ebay

An interesting discussion about using Visoflex Leica lenses on Pentax 645D can be found here.
