Did you have a hard time with focus with such a bright background? I ask because this is a problem I have.

I get my share of misses, but I don't think it's because of the background. More often it's the small movements of the model, especially if you are photographing at anything wider than f/4

Natural pose, I like the model.
I'd say 1/3 of a stop underexposed.
Do I see red eye from the flash?

No, we've been using a continuous light head that was there.
It was a tungsten temperature lamp so a lot warmer than daylight.
