Qu about Kodachrome 64 in UK

I've got one last roll of 135x36 Kodachrome 64 to use up. The mailing envelope has the Lausanne address for UK customers but I'm pretty sure this facility has closed down now and we have to send films to somewhere in USA.

Does anyone have the US address please, and any experience of what the service is like? Do I have to add extra postage cost or anything?

And is K-64 still available?
