Seems that with Canon adapter infinity is at about 25 meters.
Is there an easy fix?

I have the Б version, I don't have problem with usual P6 to M42. Do you have other lenses to test the adapter?
Still, strange that you mounted a V lens on P6 adapter, since V are not P6 mount. Maybe it was adapted, not regarding to the different focal flange?
Б/P6 = 74.10mm
V/Kiev-88 = 82.10mm _________________ Pierre

???
ok, that was new
so the (cyrillic)B = V is kiev 88 version, and I have a modified one
but 8mm is damn much difference, it is noticed but not that well, I wonder it was cheap (about 30€) _________________ MF: Kodak DCS SLR/c; Samsung NX10; OM-10; Canon T50

Maybe your adapter usualy go past infinity with normal lens + at this distance from sensor, I think mm does less difference than when lens is close to sensor. _________________ Pierre

I think it's just the opposite, at macro distances lenses move almost centimetres but from say 10 meter to infinity just fractions of millimetres.
edit: going to see if there is an k88 -> p6 adapter attached I didn't notice _________________ MF: Kodak DCS SLR/c; Samsung NX10; OM-10; Canon T50

sorry about the picture quality but it is a really hot summer day here
is this some sort of adapter or some factory conversion kit - or some local hack?
mount says KO-6/62 and an Arsenal logo
straight p6 bayonet, not k88 thread?

So it's definitly a modified V mount converted to Б with a reversing ring. See more infos here : http://araxfoto.com/accessories/reversing-ring-62/ _________________ Pierre
